Your Opportunity :
Home Care is a multi-disciplinary team with Registered Nurse, Social Worker, Respiratory Therapist, Physiotherapist, and Occupational Therapist and Rehab Assistants who collaborate and conduct home visits to deliver services to clients with a wide variety of conditions, including complex health needs.
Working in a fast-paced environment, the Physiotherapist I is responsible for providing comprehensive assessment, consultation, and treatment programs for patients in their homes.
Some interventions include strengthening, stretching, mobility, and use of modalities. The Physiotherapist I will also help instruct interventions to a Therapy Assistant and collaborate with the multi-
disciplinary team to provide effective patient care and appropriate discharge planning.
As a Physiotherapist I, you will be working both independently and as part of an interdisciplinary team providing a range of physiotherapy services to patients and families within the established standards, directives and policies for the work environment and the profession.
These services include : assessment / screening, development of customized goals and treatment plans for therapeutic interventions, delivery of therapeutic interventions, education, discharge planning and follow-
up to enable patients to achieve optimum function and independence. As a Physiotherapist I, you will include patients and families as integral members of the health care team, encouraging their active participation in all aspects of care.
You will be an integral member of our team through provision of guidance to other healthcare professionals and clinical direction to therapy assistants, support personnel and students.
You will be reviewing clinical referrals, screening and assessing client needs from a physiotherapy perspective in collaboration with other health disciplines as appropriate.
You will be developing, planning and implementing physiotherapy services in cooperation with other disciplines in the hospital and community, and establishing and maintaining effective communication with colleagues, medical staff, and health care practitioners concerning treatment plans, progress and discharge.
You will be participating in client, family and caregiver education in collaboration with other disciplines, producing and maintaining information, documentation and verbal reports to support the delivery of physiotherapy services.
You will maintain a practice knowledge base including knowledge specific to the defined service area, relevant services and resources and regulatory college standards and requirements.
Required Qualifications :
Completion of master's degree in Physiotherapy from an accredited university program or bachelor's degree in Physiotherapy from an accredited university program and related experience.
Active or eligible registration and practice permit with Physiotherapy Alberta College + Association.
Additional Required Qualifications :
Bachelor of Science Degree or Master’s Degree in Physiotherapy is required. Current registration with the College of Physiotherapists of Alberta (CPTA) required.
Current Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP) certification required.Proven ability to provide exceptional clinical care within a continually changing healthcare environment.
Basic computer skills required.Possession of a valid driver’s license and motor vehicle in good repair for work-related travel is required.
The successful candidate will work independently in clients’ homes, which could include single family dwellings, condominiums, and / or walk-
up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carrying medical bags, which weigh approximately 25-30 lbs.
Preferred Qualifications :
Experience in Home Care is preferred.Experience with Meditech is an asset.Excellent interpersonal and communication skills.
Excellent organizational and time management skills.Demonstrated ability to work as part of an interdisciplinary team.You may be required to see clients with acute, LTC, and end of life conditions.
