We are seeking several dynamic Registered Nurses (RNs), passionate about home / community nursing with experience in at least two of the following areas : chronic disease management, health promotion, palliative care and mental health care.

The successful candidate will be part of an innovative new model of care which has several key components : This position is a Full Time Salaried Contract.

With an opportunity to extend at the end of the contract.

  • You have the complete freedom to work closely with family physicians, other health care providers, families, friends, volunteers and others in the community that can support the wellbeing of your clients.

  • As a SE Health neighbourhood Registered Nurse (RN), you and your team will assign one Registered Nurse (RN) as a primary nurse for clients while supporting other clients as required.
  • As a primary nurse, you provide continuous and comprehensive support to your clients while covering for other nurses who may not be available.
  • You will conduct comprehensive assessment and care of your primary clients and their families and develop with them their plans of care based on their unique needs.
  • These plans of care will include both the short term and long term care requirements while addressing their physical, mental, social, spiritual and developmental needs.

    You will lead the process and work with other disciplines to complete and implement inter-disciplinary patient care plans.

  • As the primary Registered Nurse (RN), you will decide at what points in time other health care professionals are needed to support the client and how the care team will work in the true spirit of inter-professional care.
  • For example, personal support workers (PSWs) may be engaged to support some of the role responsibilities of the Registered Nurse (RN) and will work closely with the Registered Nurse (RNs) if a physiotherapist is required, the RN and PT will work together to establish the physiotherapy interventions and build team capacity to support the client with these interventions.

  • A unique role of the neighbourhood RN is to support the clients and their families develop strong self-care strategies, anticipate and address health issues, as well as optimize their health through effective health promotion strategies.
  • This may include developing individual and / or group based programs where none exist in the neighbourhood or are not easily accessible.

    The RNs will also work in partnership with other programs and organizations to develop health promotion, chronic disease management, palliative care and mental health support strategies.

  • Once a client / family connects with a SE Health neighbourhood team, the team will remain engaged with them even if services are temporary provided in different parts of the care continuum (primary care, rehabilitation, acute care, etc.
  • and until the point when supports are no longer required. In other words, you will provide both continuity of care and continuity of caregiver through a long term relationship with the client / family.

  • A significant part of your role also includes addressing issues related to social determinants of care that may act as barriers to wellness.
  • This means that you help the family link with and successfully engage with community and governmental supports to address issues such as social supports, housing, income and safety.

    An ongoing role of the RN and the team will be to explore, understand and develop a strong network of social care supports in the neighbourhood and broader community you serve including optimizing the client’s informal supports.


  • Registered with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN)
  • Bachelor degree in Nursing preferred
  • Canadian Nurses Association (CNA) community nursing certification is an asset
  • Minimum of two years of nursing experience
  • Having diverse nursing experiences with different client groups will be an asset
  • Experience in at least two of the following areas : home / community care, acute care, chronic disease management, health promotion, palliative care and mental health
  • Demonstrated clinical leadership experience
  • Demonstrated skills working independently and in a team environment
  • Excellent skills in case management and working with / coordinating interdisciplinary care teams
