JOB SUMMARY :

  • As an integral member of Allied Health, the Service Coordinator will : coordinate intake and admissions within the Brain and Spinal Cord Rehab Program Outpatient Services;
  • communicate effectively with referral sources, ensuring excellent customer service; manage waiting lists and communicate with patients / families on a regular basis;
  • coordinate / prioritize services across the continuum of care (i.e. inpatient to outpatient, between streams of service);
  • maintain a client focused and customer service environment; demonstrate flexibility in dealing with competing priorities and deadlines on a daily basis;
  • contribute to Continuous Quality Improvement initiatives and research; monitor performance indicators, patient satisfaction surveys, and generate reports;
  • compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.

    This position will be based at Rumsey however work at University Centre and / or Lyndhurst may be required

    QUALIFICATIONS :

  • Bachelor’s degree with a clinical background i.e., health related field
  • Member in good standing of the respective college (as applicable)
  • Minimum of 3 years experience in a patient care environment (rehab environment preferred)
  • Experience in an outpatient environment preferred
  • Experience in the field neurological injuries an asset
  • Proven abilities in project planning, implementation and evaluation required
  • Excellent computer skills required (Word, MS Outlook and Excel required, Access preferred)
  • Excellent interpersonal skills
  • Knowledge of standards of practice and applicable health care legislation
  • Experience working with inter-professional teams required
  • Self directed individual with the ability to work both independently and in a group setting
  • Demonstrated leadership abilities and experience required
  • Excellent decision making skills required
  • Excellent verbal and written communication skills required
  • Excellent organizational and time management skills required
  • Successful completion of Basic Life Support (C level) Heart Saver plus (Canada)
