We are currently seeking :
Community Support Workers Durham Region
VHA’s Community Support Program provides support to clients in the community, with compassion and with clinical responsibilities.
The DCSW is responsible for providing emotional support, assisting individuals that have physical and / or mental health challenges, and encouraging them through a client centered approach.
Assist the clients by connecting them with support programs and services available in the community and guide them through different social activities.
Required to assist the supervisor with a variety of administrative tasks to support the program including scheduling, tracking documents and community engagement.
All activities are conducted within the clients care / service plan and policies, procedures and standards of confidentiality of VHA Home HealthCare.
May be required to wear Personal Protective Equipment (PPE) and assist with sanitization of program areas.
Key Accountabilities :
Communicate with local social service agencies to connect clients to supports and services within their communities
Provide high quality service in line with the values and principles of the program by exercising good judgment and consulting with Supervisor
Work under the direction of the supervisor, providing harm reduction strategies and community support.
Conduct street outreach to locate un-sheltered / at-risk populations to develop trusting relationships and to make support connections
Develop and facilitate support programs according to the needs of clientele and to identify gaps in service. May include cooking, cleaning, handling of harm reduction materials
Key knowledge, Skills, Abilities, and other attributes :
Diploma in Social Service Worker, Community Service Worker, Mental Health and Addictions or a related discipline
2-3 years of experience in a social service setting
Lived experience within the homeless sector highly recommended
Strong knowledge of Durham Region Social Service agencies
Working with people that are unsheltered, that have physical and / or mental health and addiction issues, high-risk population
Assessing client needs and connecting clients to community supports (ID clinics, doctor, case management etc.), working with clients to develop care plans
Developing and facilitating support programs (cooking, life skills, budgeting)
Community outreach
Assisting clients in finding suitable housing
Liaising with community service agencies and health care professionals to develop one-stop shop for supports and services
Maintain client files and data entry
Excellent interpersonal skills; demonstrating tact and diplomacy with at risk clientele
Demonstrated interest in mental health and homelessness issues
Excellent problem-solving skills, a solution-focused approach and crisis intervention techniques
Highly organized and comfortable working in a constantly shifting environment
Demonstrated ability to resolve conflict and develop strategies for positive outcomes
Experience working with clients with mental illness
Able to work from a client centered and harm reduction approach
Computer skills in word processing, data entry, and Microsoft Office Suite programs (Excel, Word, Outlook)
Ability to successfully complete the Physical Readiness Test
Demonstrates efforts to continuously develop and upgrade skills
Ability to travel from client to client in the community as required
A valid driver's license and access to a vehicle is required
