The Canadian Red Cross Society, a not for profit, humanitarian organization dedicated to helping Canadians, as well as the most vulnerable throughout the world, is currently seeking a PATH Assistant, within the Sudbury Branch PATH program, and under the direction of the Senior Manager of Operations.
Duties include but not limited to :
Provides support to seniors during hospital discharge process
Supports clients during the transition from hospital to client’s home in accordance with established service plan
Completes basic errands for clients as required, in accordance with established service plan (i.e. grocery pick up, etc.)
Completes basic home management duties, in accordance with established service plan.
Completes risk analysis of home using standardized Risk Assessment tool
Facilitates client and caregiver engagement., i.e. providing applicable information on client related issues
Facilitates referrals to Community Support Services and discharge process as per program guidelines
Handles and stores hazardous products safely (WHMIS)
Maintains comfort and safety of client’s immediate environment
Identifies and reports any unsafe or hazardous equipment and situations to the Program Coordinator
Takes measures to ensure personal safety
Takes measures to protect the health and safety of clients
Observes and reports any concerns related to the client or their environment to the Program Coordinator
Perform other related duties as assigned and required; in consultation with the Program Coordinator
Provides companionship to clients being transitioned home
Maintenance of confidentiality of clients
Assist Program Coordinator with statistics and reports as required
Qualifications :
Must be between 15 and 30 years of age (inclusive) at the start of employment
Excellent interpersonal and recording skills
A caring, non-judgemental individual
A self-motivated individual who takes initiative
Proficient in troubleshooting and resolving problems
Bilingualism an asset
Good writing and verbal communication skills
Standard computer knowledge (Microsoft Office and Internet Research)
Must be returning to school in the fall
