Roles & Responsibilities

Job Description

  • To assist the Senior / Staff Nurse in the Clinic and provide clinical care using specialist knowledge for persons with ID in health promotion and education
  • To work with primary care teams, community health partners and with hospitals to provide holistic services
  • To understand the health care issues of persons with ID and help in the planning process of programmes
  • To act as case manager to support the persons with ID and to provide person-centred care
  • To be involved in home-based assessments
  • To help families access health care services
  • To be involved in follow up of referrals
  • To assist Senior / Staff Nurse in making / changing medical appointments for clients when applicable
  • To assist Senior / Staff Nurse in attending to any medical problems / emergency of clients
  • To assist Senior / Staff Nurse in developing and maintaining of procedures, protocols in nursing standards and documentation of medication administration, if any
  • To maintain and update documents for MOH inspecting / licensing
  • To assist in setting up and maintenance of training materials and resources
  • To attend and participate in staff meetings and debrief
  • To perform any other duties assigned by Reporting Officer
  • Job Requirements

  • Good interpersonal and communication skills
  • Computer literacy skills MS Word and PowerPoint
  • Experience working in the social services or health sector preferred
  • Demonstrated ability to work effectively and efficiently with minimal supervision
