Employee Performance Rewards Responsibilities
Provide short term, post acute interventions as well as long term care for clients in the home or in care facilities
Care and support may involve basic care to more complex care requiring highly specialized clinical interventions including health promotion, acute and chronic care, end of life care, rehabilitation etc.
Informs the Director of Nursing and any other appropriate personnel of changes in the client’s conditions and needs
Counsels client and family through care process while updating family on a regular basis
Assures proper recording and maintenance of clinical records in compliance with company quality standards
Participates in agency performance improvement activities and continuously staying informed about changes in the field of nursing and home health care Requirements
Graduated from an accredited and recognized registered practical nursing program
Current license as a Registered Practical Nurse with CNO, in good standing.
Minimum 1 year of nursing experience required in an acute care / homecare setting
Current CPR certificate
demonstrate strong clinical skills
Valid Driver's License and automobile insurance
TB test or chest x-ray
