Summary of Duties
To provide a full range of social work services to patients and their families at the Holland Orthopaedic and Arthritic Centre.
This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation.
Qualifications / Skills
Masters Degree in Social Work
Member of the Ontario College of Social Workers and Social Service Workers
Minimum two (2) years of experience working in a Hospital setting
Previous experience working with musculoskeletal surgery, geriatric / elder care and end of life
Cognitive support experience required
Excellent assessment, interviewing and problem solving skills
Excellent written and oral communication, mediation, time management and organizational skills
Strong team building skills
Knowledge of the health care system and community resources
Superior ability to manage multiple tasks and set priorities in a fast-paced environment
Commitment to person centred care
Commitment to student education Exceptional communication skills required
Familiarity with issues of capacity and consent
In-depth understanding of the Sunnybrook Discharge Policy and its application
Strong negotiation and conflict resolution skills
Ability to be a team player as well as self-directed
Ability to manage multiple tasks and set priorities in a fast-paced environment
