  • Be a part of the team leading the nation in healthcare.Location3081 Heartland Health Care Center Moline, IllinoisEducational RequirementsBachelor's Degree from an accredited school of Social Work, or Licensed RN, or Recreational Therapist required;
  • Master's degree preferred; Experience in dementia care and services; Positions Licensed as required by law.Position RequirementsOne year of experience in health care in an Alzheimer's / Dementia setting required.

    Job Specific Details : Participates in the initial pre-admission evaluation of potential patients.? Plans and coordinates the unit?

    s therapeutic program schedule which meets the spiritual, social, emotional,physical, and intellectual needs of the patients.

    In conjunction with other departments, plans and organizesprogram content.? Informs and consults with staff regarding the unit?

    s program and integrates the program with other services asappropriate. Works collaboratively with the Administrator and Director of Nursing.

  • Monitors daily functioning of the unit to ensure continuity of, and appropriate changes in, the program.? Assesses the educational need of unit staff regarding dementia specific knowledge, behavior management,communication skills, strategies to support patient function, and knowledge of the program.
  • Works with the staff member responsible for in-service training to plan, coordinate and conduct appropriatetraining for unit staff;
  • Provides in-service training relevant to the needs of center personnel, as directed by theAdministrator, regarding social / emotional needs of Alzheimer?

    s residents.? Conducts appropriate reviews of unit services to ensure compliance with program standards.? Treats patients with respect and dignity at all times.

    Serves as a role model for staff regarding care of dementiapatient.Communicates immediately to the Administrator information concerning suspected, alleged, or actual patientabuse of a physical, verbal, mental or sexual nature committed by employees, visitors, or patients.

  • Maintains accurate and timely documentation which complies with federal / state regulations, HCR Manor CareCorporate policy, and specific center practices.
  • Establishes and maintains a sense of teamwork through effective communication, interaction, and team meetings;Develops and maintains a cooperative relationship with other department heads;
  • In cooperation with Admissions / Marketing, markets the unit through involvement in community organizations andparticipates in the local Alzheimer?
  • s Association.? Provides family members with information about the unit, for example, through the center?s newsletter or othercenter communication.

  • Provides assistance with patient admission to ensure a smooth transition.? Assists with the adjustment of the newly admitted patient and family to the center;
  • Contacts the new patientand / or family weekly for the first month after admission to ensure that needs are being met and that difficulties arebeing resolved.

  • Ensures appropriate discharge plans are available upon admission and revised as necessary; When appropriate,consults with and assists the patient and family in planning and implementing the patient?
  • s discharge;? Keeps abreast of current research, new programs, and community resources which may benefit patients andfamilies, and makes appropriate referrals to other consultants, community agencies, or center departments inorder to facilitate a patient?

    s maximum use of resources, and to promote a patient?s appropriate level offunctioning.? Assists patients in the maintenance and adequate supply of personal clothing and other personal items.

  • Advises patient of his / her right to vote, and provides assistance with voting, if requested.? Encourages patient autonomy by promoting attendance at resident council meetings.
  • Informs the patient, family, or legal representative about center related services that are available upon request.? Informs patient, family, or legal representative of Medicare / Medicaid program benefits and assists with applicationfor these alternative funding programs for nursing home care.
  • Education? Licensed registered nurse, social worker or certified recreation therapist required.Experience? One year of experience in health care in an Alzheimer?

    s / Dementia setting required. Supervisory experiencepreferred.
