Reporting to the Director of Care, the Licensed Practical Nurse is the role model and team leader, assigned to a unit, responsible for coordinating care delivery.

The Licensed Practical Nurse performs duties and monitors departmental activities in accordance with their scope of practice, established standards, regulations, policies and procedures to maximize individualized and resident focused care.


  • Include but not limited to;
  • 1. Assess residents’ needs; plans, implements and evaluates resident care.

    2. Ensures quality, individualized, resident focused care is provided and resident rights are observed. Assists residents with care and lifts / transfers when necessary.

    3. Encourages and adheres to a holistic philosophy of wellness, recognizing that each individual is capable of, to varying degrees, making decisions about his / her life and is a partner in the decision-making process.

    4. Coaches and mentors care staff using in the moment education to improve resident care. Seeks assistance / support from RN or ADOC as needed.

    5. Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality focused resident care is delivered.

    6. Provides work direction to care staff as needed, including reassigning staff when necessary to meet resident care needs.

    7. Ensures care staff complete all POC documentation, providing coaching as needed.

    8. Assesses and treats all wounds on the unit. Consult with RN for changes in treatment strategies as needed.

    9. Acts as a resource to care staff for strategies to prevent skin breakdown.

    10. Has a Primary LPN role, responsible for an assigned group of residents. Some Primary Nurse responsibilities are care planning, quarterly and annual RAI assessments, regular communication with families.

    11. Administers prescribed medications and treatments within the limitations of the BCCNM (BC College of Nurses and Midwives).

    Monitors and reports changes in physical and psychological conditions of residents and documents changes in the resident health record.

    Notifies family and physician of changes in condition.

    12. Active participant in Interdisciplinary Care Conferences, ensuring all assessments and documents are complete prior to the Care Conference.

    Also responsible for any follow up referrals and care plan changes resulting from the Care Conference.

    13. Participates in resident admissions according to facility guidelines and admission policy.

    14. Maintains all documents according to established standards, regulations, policies and procedures, ensuring confidentiality is maintained according to Facility policy.

    15. Actively participates in orientation of new staff, facility programs, meetings, committees and educational in-services.

    16. Ensures equipment and supplies are stored and maintained according to policy and procedure and reports unsafe or faulty equipment to appropriate manager.

    17. Maintains an up-to-date knowledge of facility policies and procedures, particularly those concerning the administration and recording of medications, safety and emergency procedures.

    18. Utilizes the Registered Nurse as a resource for complex situations as per the policy of the facility.

    19. Performs other related duties as required.


    1. Graduate of an approved program for Practical Nurses and holds a current License with the BC College of Nurses and Midwives (BCCNM).

    2. Valid OFA1 certification.

    3. Successful annual RAI competency testing.

    4. Two years recent experience as a practicing LPN in long - term care that includes the administration of medications and giving work direction in a team leader role.


    1. Ability to read, write, and speak English proficiently and understand verbal and written instructions.

    2. Computer skills including the ability to navigate basic computer programs such as Word, Excel, Outlook.

    3. Conflict resolution skills.

    4. Excellent communication skills

    5. Ability to operate related equipment.

    6. Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand and walk as required to perform the duties of the position.
