Reporting to the Transitional Care Unit Manager, the Clinical Lead will assist the Manager to build and strengthen working relationships between both regulated and unregulated team members.

A strong focus will be placed on ensuring daily operations run smoothly and efficiently. Working with the TCU Manager, the Clinical Lead will provide leadership support in a dynamic environment during evenings and weekends, supporting an inter-professional team, and collaborating with key internal and external stakeholders.

The Clinical Lead will work closely with ICS Leadership and front-line staff on continuous quality improvement initiatives as well as ensuring patients receive high quality care.


  • Review patient care plans and documentation as appropriate; meet with patients as required and ensure staff are following care plans as appropriate.
  • Supervise and support regulated and unregulated professionals to ensure compliance with all legislated standards of practice as well as company policies and procedures.
  • Ensure that appropriate infection prevention and control measures are in place and followed at all times. In addition, ensure staff have required personal protective equipment required to fulfill the expectations of their role.
  • Assist with the integration of client satisfaction initiatives; ensure that all patient concerns and complaints are satisfactorily addressed and documented in a timely manner.
  • Work with Manager to ensure that medical equipment is properly cleaned and maintained as well as ensure the Unit inventory of medical supplies is adequately maintained.
  • Follow company policies and procedures and attend Clinical Quality events and training as required.
  • Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body.
  • Work with Manager to coordinate the in-service education program for field employees and maintain appropriate records.
  • Support the Manager to ensure that relevant policies and procedures are followed by staff.
  • Assist with orientation and training of new staff, discipline, and the probationary / annual performance review of reporting field employees
  • Assist with the implementation of Modified Work Programs for Field employees
  • Attend and participate in operational or external meetings as requested.
  • Promotes effective teamwork and interprofessional collaboration in delivery of effective patient care.
  • Support the ICS Leadership team to implement organization wide and clinical program priorities and initiatives.
  • Promote Bayshore ICS through participation in local healthcare or business committees as well as community events.
  • Participate in ongoing internal and / or external continuing education activities.
  • Adhere to Bayshore Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participates in Health & Safety activities and ensure that accident or injury reports are completed within 24 hours of occurrence.
  • Maintain confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Demonstrate solid performance or exceed performance standards in key job dimensions / attributes as defined on the Performance Appraisal.
  • Complete other tasks as requested.

  • Maintaining Continuing Education Requirements (CEUs) as required by regulation.
  • Demonstrated record of building and maintaining stakeholder relationships.
  • Demonstrated knowledge of the Canadian health care system including OHIP, insurance & private pay.
  • Previous discharge planning experience preferred.
  • Proven ability to provide excellent customer service.
  • Demonstrated ability to work well in a team environment
  • Working knowledge of business office technologies : PC / laptop with Microsoft Office applications, smartphone, website / internet, email.
  • Ability to read, write and effectively communicate in English.
  • Must be willing to travel and work evenings, nights and weekends as required
  • Demonstrated progressive and relevant experience in healthcare with increasing complexity and scope; experience working in a LTC setting is an asset.
  • Demonstrated ability to think strategically and implement strategies.
  • Experience working with diverse clients and stakeholders.
  • Strong organization and planning skills required.

  • KEY QUALIFICATIONS : Education :

    Education :

    Current license in good standing as a Registered Practical Nurse in the province of Ontario.

    Experience :

  • 3+ years’ experience in health care, preferably hospital, long-term care or retirement home
  • 3-5 years of leadership experience

    The Clinical Lead reports to the TCU Manager, as appropriate


    The Clinical Lead must demonstrate ongoing competency in completing all duties and responsibilities of this job description, in response to changes by the regulating body, as well as agreed upon specific goals and objectives.


    Performance will be reviewed on an ongoing basis with a formal review at the end of the six-month probationary period and subsequently, on at least an annual basis.

    Input will be sought from employees and consultants at the branch and National Development Centre as applicable.
