Job Summary :

Reporting to the Nurse Manager, works as part of a team to facilitate client care with front line staff and Continuing Care Coordinators.

This position will provide supervisory support to the Home Support Services program. Relationship building with funders and Home Support Workers is essential.

The Service Delivery Supervisor collaborates with Nursing Managers on issues that require direction. Care is provided within the framework of quality management and the standards and scope of practice set by VON, Continuing Care and the College of Licenses Practical Nurses of Nova Scotia .

Responsibilities :

  • Conduct initial visits for new referrals as per Home Care Standards
  • Performs supervisory and client specific visits, as required, respecting client’s rights and maintaining privacy and confidentiality
  • Contributes to CCA performance development plan
  • Ensures adequate communication with Continuing Care including but not limited to progress notes and discharge reports
  • Provide education to CCAs as needed, including orientation
  • Ensure accurate documentation in the client record is appropriate and timely in accordance with VON documentation, approved abbreviations and privacy policies
  • Ensures documentation of all client incidents in the ERS, completes and documents follow up and collaborates with managers / others to resolve issues and ensure best practice
  • Communicate with Care Coordinators as appropriate including attending case conferences
  • Possess knowledge of VON policies and procedures, and applicable legislated Acts and regulations
  • Leadership :

  • Works collaboratively within a multidisciplinary team and acts as a clinical resource and mentor for CCA’s / HSW’s
  • Shares knowledge, enabling clients, family, caregivers and community to make informed choices in determining health and service needs
  • Promotes involvement of client / family / caregiver in determining health and service needs
  • Quality & Risk Management :

  • Practices in accordance with the quality and risk management framework of VON Canada and Occupational Health and Safety legislation
  • Articulates knowledge of components of quality and risk management
  • Plans, implements, evaluates and adjusts care in accordance with customer focused expectations
  • Seeks input from clients to assure that service meets expectations
  • Functions as an effective team member
  • Participates in decision-making and follows up to promote continuous quality improvements
  • Professional Accountability :

  • Functions in accordance with the Standards of Nursing Practice from the appropriate provincial regulatory body
  • Functions in accordance with the VON policies, procedures and strategic direction of VON Canada and the site
  • Maintains competence through continuing education
  • Collaborates with management staff and colleagues in the evaluation of own practice
  • Health and Safety Accountability :

  • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and VON policies and procedures
  • Functions in accordance with VON's risk management policies, procedures, practices and guidelines designed to promote and protect client safety
  • Participates in health and safety training including WHMIS and applies this knowledge in the workplace
  • Wears or uses any / all protective equipment or clothing provided by the VON and does not interfere with protective devices so as to change or negate their functions
  • Reports the absence of or defect in any equipment or protective device of which he / she is aware and which may endanger him / herself or another worker
  • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
  • Does not use or operate any equipment, machine, device or thing, or work in a manner that may endanger him / herself or any other worker
  • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and VON policies and procedures for workplace injuries and incidents
  • Knowledge / Skills / Abilities :

  • Demonstrated skills in leadership, community nursing, team building, communication and education
  • Understands and uses effective communication methods / tools
  • Computers skills including but not limited to Microsoft Office, Procura, Excel and Cell Trak within the work of redesign
  • Ability to work independently and collaboratively as a member of intra / inter agency teams
  • Ability to provide leadership within the organization
  • Ability to accept and facilitate substantial change
  • Ability to work under pressure and manage multiple projects simultaneously
  • Maintains relevant competencies and participates in annual reflective practice
  • LEAN" knowledge an asset

  • Understanding of the role, activities and limitations of the CCA
  • Familiarity and understanding of the role of the Provincial Continuing Care Program
  • Qualifications :

  • Registered with College of Licensed Practical Nurses of Nova Scotia
  • Recent experience working in community based health care system
  • Requirements :

  • Available to work flexible hours
  • Must have own vehicle
  • Possession of a current driver’s permit and proof of vehicle insurance
  • Current CPR Certification
  • Current Criminal Records Search
  • Must be able to wear Personal Protective Equipment (PPE)
  • Work Conditions and Physical Capabilities : Attention to detail; Lift and carry using appropriate lifting techniques; Bend, push, pull, grip, reach, kneel, crouch using good body mechanics;
  • Walk, sit, stand, climb stairs; Hand movements.
