Quality, Safety and Patient and Family Experience

Position Description

Based in the Quality, Safety and Patient and Family Experience Portfolio, the Patient Engagement Facilitator will bring the patient perspective and voice to organization processes, operations, planning and evaluation of healthcare delivery at CAMH with a focus on improving the quality and safety of patient care as well as the patient experience.

QSP / FE is seeking three (3) full-time, permanent Patient Engagement Facilitators to help incorporate a patient perspective into clinical program activities and elevate the patient voice and unique perspectives.

Reporting to the Manager of Patient / Family Experience, and working as part of a team, the candidates will facilitate, collect and integrate the feedback of patients.

Drawing from lived experience of substance use and / or mental illness and ongoing patient engagement activities, the Patient Experience Facilitator will enhance CAMH’s understanding of patient experiences and patient engagement opportunities.

Specifically, the role will include :

  • Co-Facilitate Patient Advisory Committee meetings for the Program where they are assigned
  • Lead Patient Advisory recruitment efforts within their Program
  • This includes :
  • Actively seeking recommendations from providers and other care team members
  • talking to patients who are identified as potential Patient Partners
  • Marketing the Patient Advisory Committee to staff and patients in their clinical program
  • Present PAC updates to Program Staff
  • Program Quality Council membership where you will partner, present feedback and support continued quality improvements in patient engagement and experience, quality and safety
  • Communicate topics identified by staff that they would like to present to the PAC groups
  • Support Patient Partners on various committees and working groups across the hospital
  • Provide Advisor feedback on the implementation of the Patient Engagement Workplan
  • Celebrate and advocate for integrating the patient voice into the work that we do across CAMH
  • Contribute to the development of presentations, workshops, on-line and print resources.
  • Provide consultation and recommendations on proposals, reports and manuscripts.
  • Active involvement in supporting engagement of patients in the annual patient survey (OPOC-MHA) and other patient experience measures
  • Other duties, including administrative tasks related to patient engagement activities.
  • The candidates will support a healthy workplace that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements.

    This position is located at the Queens Street Site, although some of the work may take place in community settings. Evening and / or weekend hours and travel across Canada may be required.

    Qualifications

    The successful candidates will have completed high school and / or special courses. Post-secondary education preferred.

    These candidates will have lived experience as a recipient of mental health and / or addictions services, and will possess communication skills that inspire hope and empowerment.

    They will also have a strong understanding of the principles of self-care, self-determination and issues facing patients and their families.

    These candidates will embrace the principles of Patient-and Family-Centred Care (treat all with dignity and respect, actively share and create opportunities for information-

    sharing, work in true partnership with patients and families and identify areas for collaboration). The candidate / s will possess demonstrated knowledge in the areas of presentation, group facilitation and working with patients.

    Knowledge and / or experience in quality and patient safety within hospital environments will be considered an asset.

    These positions require superior interpersonal and communication skills to work effectively and strategically with patients, staff and internal and external stakeholders from diverse backgrounds.

    An ability to work effectively as a team member in a dynamic and a fast-paced inter-professional environment is essential, and successful candidates will possess strong organizational and time management skills.

    Familiarity with MS Office, e-mail is required.

    Bilingualism (French / English) or proficiency in a second language would be considered an asset.

    Please note : These three (3) full-time, permanent positions are NOT part of any bargaining unit.
