Perform all duties inaccordance with the Standards of Practice of the College of Nurses of Ontario in providing direct and indirect nursing care as required in collaboration withother health care workers.

QUALIFICATIONSAND EDUCATION REQUIRED FOR THIS POSITION :
Graduateof an approved Registered Nursing program
Registeredwith the College of Nurses of Ontario and currently in good standing
Demonstratesstrong positive personal and interpersonal communication skills, both verballyand written. Ability to work as a memberof a multi-disciplinary team .
Current CPR and First Aid certification willbe required
Ideal candidates will have Fundamentals of Palliative Care . In addition, Enhanced Fundamentals and theComprehensive Palliative Education Program (CAPCE) and will have or be pursuingtheir CNA certification in Hospice Palliative Care Nursing.
Minimum of 2-years of experience in nursing, knowledge of hospiceand palliative care is essential
Strong clinical practices with ability to championevidence-based best practices particularly in pain and symptom management andend-of-life care
Demonstrated excellent problem-solve skills with ability tomanage complex clinical situations and psycho-social circumstances in providingor managing the needs of palliative patient’s across the continuum ofend-of-life care
Ability to recognize, assess and treat end-of-life careemergencies and psychosocial distress
Ability to recognize safety issues, follow safe work practices,advocate for client care and safety concerns.
Demonstrates a patient / family-centred approach to nursing care.Recognizes the importance of establishing authentic and empathetic professionalrelationships with patients and families.
Ability to understand and document patient and familypreferences, goals of care, and act as an advocate to help patients meet theircare needs.
Ability to collaborate with other members of the health careteam to access required services and to ensure smooth transitions
Demonstrated ability to use knowledge-transfer techniques tosupport practice delegation in addition to completing ongoing health teachingwith both patients and families.
Demonstrated commitment to modelling lifelong learning andprofessional growth
PHYSICAL DEMANDS :
Whileperforming the duties of this job, the employee is regularly required to stand,walk and assist with patient lifts / repositioning.
The employee is frequently required to reachwith hands and arms, stoop, kneel and / or crouch. The employee is occasionallyrequired to sit, climb or balance.
The employee must follow transfer and liftpolicies for safe practice, but also be able to lift and / or move up to 75 Ibs.
