    Job Description :

    Trillium Health Partners (THP) is seeking an experienced Project Manager to join the Health Hubs team. In this role, the Project Manager will have an opportunity to play a key role in the hospital’s initiative to develop a series Health Hubs throughout the communities of Mississauga and West Toronto.

    Health Hubs aim to integrate and co-locate a continuum of population specific services built around the needs of patients and are a key priority for the Board of Directors.

  • The initial Health Hubs development, a Seniors Health Campus, focuses on the needs of older adults and includes : a 220 bed long-term care home;
  • a Seniors’ Hub that will deliver services such as day programs, ambulatory / outpatient services, diagnostics, primary care, community services and retail;
  • other developments, such as retirement / assisted living as well as a third party partnership with a hospice. In this role, the Project Manager will be required to work with and / or support other departments within the hospital, including Patient Care Services and Operational Effectiveness to advance shared opportunities and capacity initiatives.

    In this role, the Project Manager may be assigned other projects within the Strategic Projects Portfolio, which may include initiatives related to advancing THP’s Strategic Plan, Major Projects (such as hospital development and capacity planning) and / or Partnership Projects.

    Accountabilities :

  • Ensure alignment to Trillium Health Partners’ strategic plan and core priorities;
  • Lead the planning, coordination, communications and stakeholder engagement processes for Health Hubs initiatives;
  • Oversee all elements of project approach and plan development, as well as implementing all necessary; processes and practices to ensure the hospital is delivering project objectives and successfully achieving the project's goals;
  • Lead and / or support the development of materials, such as reports, briefing notes, requests for proposals, presentations, data analyses and communications for the project(s) for all levels of the organization’s partners, government and external partners, and the Board of Directors;
  • Lead the development and dissemination of project control documents, including status reports, risk / issue analyses and reports and project plans;
  • Monitor and report on progress, problems and solutions of the project to all stakeholders and escalate issues as appropriate;
  • Engage with external project supports and act as a key point of contact for coordination of activities;
  • Collaborate with THP’s analytics team(s) to define capacity requirements and opportunities to support multi-year planning that will be enabled by Health Hubs and other capacity initiatives across the organization;
  • Build trusted relationships with third-party partners to advance key initiatives;
  • Provide strategic oversight and guidance to the governance structure established to oversee the project’s deliverables;
  • Lead and / or support working groups that are integral to the success of the project(s);
  • Support key committees as required;
  • Engage and communicate with stakeholders on an ongoing basis and ensure changing resource and project requirements are met and understood;
  • Support multi-disciplinary project team through all phases of the project life cycle, providing coaching, motivation and guidance throughout the duration of projects;
  • Liaise with internal and external stakeholders including regional, provincial and / or private sector partners to ensure the successful completion of project deliverables;
  • Support project research, literature reviews and data / information gathering to enhance subject-matter knowledge relevant to the project; and
  • Lead or support other projects, as required within the Strategic Projects Portfolio, which may include initiatives related to advancing THP’s Strategic Plan, Major Projects (such as hospital development and capacity planning), virtual care and / or Partnership Projects, as well as projects THP may engage on with external partners, such as the MOHLTC.
  • Other duties assigned, as required to meet timelines and deliverables
  • Qualifications :

  • Undergraduate degree in business, life sciences, health informatics or related discipline required
  • Masters degree in public administration, business administration, or healthcare administration or equivalent experience an asset
  • Minimum 5+ years of experience in project management in healthcare environments such as hospitals, long-term care, government or private sector required
  • Extensive knowledge of provincial health care structures, processes, funding mechanisms and priorities is strongly preferred
  • Experience in or with long-term care and seniors health services is strongly preferred
  • Experience with planning for Health Hubs an asset
  • Exceptional relationship building abilities
  • Strong understanding of the hospital’s strategic plan
  • Strong understanding of THP’s challenges (internal and external) and familiarity with capacity planning analytics, solutions and partnerships
  • Experience with project management processes and best practices, as well as corporate level reporting
  • Experience with effectively engaging with physicians, clinicians, senior leaders, executives and Boards of Directors required
  • Experience effectively engaging with and managing third-party partners and delivery partners required
  • Experience with government relations is an asset
  • Excellent organizational, time management and project management skills
  • Demonstrated ability to think proactively and to anticipate needs of senior leaders
  • Demonstrated ability to multi task and prioritize tasks effectively
  • Superior analytical and problem solving abilities
  • Superior data analysis and manipulation skills
  • Working knowledge of financial modeling and financial analysis
  • Demonstrated effectiveness at leading and working with teams as well as working independently
  • Demonstrated effectiveness in managing ambiguity
  • Ability to work under pressure and tight timelines
  • Demonstrated innovative and out-of-the-box thinking
  • Experience organizing and facilitating large executive level retreats with multiple stakeholders
  • Effective interpersonal communications skills and ability to successfully build relationships
  • Demonstrated ability to develop effective and comprehensive reports and presentations for various audiences, including executives, administrators and clinicians
  • Superior writing skills
  • Strong working knowledge of computer applications, especially the Microsoft Office suite
  • Satisfactory performance and attendance required
