ASSOCIATE DIRECTOR OF CARE, REGISTERED PRACTICAL NURSE

Position Overview :
As a passionate leader, you will assist and oversee the nursing program for the community. As an expert in the industry, you are able to strategically guide and support a team of talented healthcare professionals who have a strong desire for delivering exceptional care.
As a specialist in your field you have the ability to manage multiple priorities and the complex care needs of residents.
Co-ordinates directs and monitors the effectiveness of all operational activities within the nursing department;
Participates in the implementation of an effective quality and risk management system for the nursing department to ensure legislative regulations and standards are met;
Monitor and evaluate that nursing unit team members are compliant with the organization’s policies and procedures and RHRA standards;
Manages and coordinates the planning, implementation, and evaluation of a wide range of external consults including CCAC, LHIN’s, and community stakeholders such as Hospitals, Home Care Agencies and Senior Community Centres as well as doctor’s offices.
Establish, maintain, and communicate current resident assessment schedule and expectations to the inter-professional team;
Care Plan and review implementation
Facilitates communication in complex care issues to foster positive resident outcomes;
Promotes an effective admission and discharge process and experience
Ensures that all risk areas identified are addressed in a timely manner and according to support services policy and applicable legislation RHRA and MOH
Work in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act;
Knowledge of RHRA standards and procedures
Establish and maintain cordial relationship with families
Staff performance evaluation
Attends off-site meetings
Must be available for flexible hours (including weekends and evenings as required, or when acting as the Manager on Duty on certain weekends);
Registered Practical Nurse with a current diploma in good standing with the College of Nurses of Ontario;
Relevant management and leadership experience in healthcare, retirement or long-term care facility, or head nurse in chronic care or senior management in a health community agency;
3+ years of experience working in clinical nursing, long-term care, retirement homes, or chronic care settings;
Current Basic Cardiac Life Support Certification (BCLS)
Must have an extensive knowledge of complex healthcare resident requirements;
Must be able to multitask in a fast-paced changing environment
Must be prepared to respond to emergency situations
