We are currently seeking part-time or full-time Service Coordinators for our Markham Service Delivery Centre. The Service Coordinator will be primarily responsible for the efficient and effective coordination of client care.

This includes scheduling staff, updating and maintaining electronic client records, data entry of billing information, responding to all caller inquiries and ensuring they are dealt with appropriately.

Receive client referrals, obtain required client information and schedule CHWs in accordance with care plans and applicable agreements,

Communicate with clients regarding placement, including staff member names, times of service and changes to the schedule,

Provide information to CHWs regarding placements, including client name, time of service, and changes to schedules. Provide direction regarding the implementation of client care plans,

Communicate changes in the client care plan and client condition to CHWs and agency nurses as required,

Communicate changes in client condition, the physical environment and family status to the service purchaser (e.g. case managers) and implement new care plans as authorized by the service purchaser.


Certificate in Office Administration, Medical Administration ( Asset)

Customer Service Experience

Recent or related experience

Or an equivalent combination of education, training, and experience or other Qualifications determined to be reasonable and relevant to the level of work

Ability to communicate effectively, both verbally and in writing

An ability to work independently and in cooperation with others

Ability to plan, organize, and prioritize

Ability to type at 50 wpm

Business writing skills

Ability to analyze and resolve problems
