POSITION SUMMARY :
In collaboration with an interdisciplinary team of health professionals and Saint Elizabeth Leadership team. The RN Supervisor is responsible to support the LPN supervisors, Community Health Workers and Client Service Coordinators within the RN scope of practice with the objective of ensuring the quality and safety of client care and client's environment.
As well, the RN supervisor is responsible for training and ongoing monitoring of Community Health Workers (CHWs) in the provision of safe personal care to clients and developing and monitoring related techniques and procedures.
Ensure timely follow-up with clients and their families
Ensure highest level of care is maintained.
Facilitate orientation of new CHWs and provides additional onsite training for CHWs as needed.
Collaborate with the management to meet benchmarks for operational efficiencies.
Actively participate as a team member in project / program development and implementation.
Liaise with Home Care Nurses and Case Managers regarding client care.
Initiates / attends client / family conferences with HCN and CM to address and resolve client issues.
Initiates and facilitates meetings with CHW to discuss and resolve challenging client care issues.
Collaborates with other nurse supervisors regarding task delegation to CHWs
Reports directly to the clinical Nurse Manager
Attends potential clients’ discharge planning meetings with health unit case managers and Home Care Nurses.
Performs complex nursing procedures for community clients.
Follows up and investigates worker injuries and RL6 incidents.
Conduct initial follow up with CHWs regarding performance issues.
Additional duties as required.
A Registered Nurse designation (CRNBC)
Previous community health care experience or at least 2 years acute and long term care experience as a Registered Nurse
Comfort level in promoting SE Health services both to individuals & health care partners.
Teaching experience is an asset
A current CPR for HCP and First Aid certificate
Excellent interpersonal and verbal / written communication skills
Excellent organizational, supervisory and time management skills
An ability to deal with conflict and excellent problem solving skills
A vehicle and valid driver's license and the ability to travel
High level computer proficiency
Day hours, with flexibility in covering evenings and weekends
