Seeking : Coordinator of Transitional Care Team, a privately-funded program to help graduates of the Jewish General Hospital Child Psychiatry Day Hospital.
The program serves families whose 5-12 year old children received 6-12 months of intensive treatment at the Child Psychiatry Day Hospital.
The Transitional Care Team assists children after hospital discharge to reintegrate into their schools and communities and seeks to maintain improvements and prevent re-
admission into Child Psychiatry services.Job Duties :
Provide support, coaching, liaison and referrals to parents of children who have been discharged from the Child Psychiatry Day Hospital
Supervise a team of two childcare workers who observe discharged children in their community schools and provide support to school personnel.
Collaborate with Jewish General Hospital Child Psychiatry staff prior to and after discharge of patients; this includes family and school meetings at the time of discharge
Evaluate discharge needs of children, families and schools.
Provide weekly telephone support and coaching to parents, and home visits where warranted.
Attend school meetings when needed
Document and record all interventions
Report to the team supervisors
Requirements : Knowledge and Skills :

Knowledge of child development and childhood psychiatric disorders
Experience working with families of high risk children with complex psychopathology
Organized, flexible individual who can work independently and provide leadership for the team
Knowledge of Jewish General Child Psychiatry Day Program an asset
Ability to work with families and teams in varied settings, including hospital, schools and homes of familiesEducational Qualifications
Masters in Social Work, Counselling or equivalent; member of the Order of Social Workers or other professional order in Quebec that offers insurance coverage.Salary and Hours
