As a result of our continuous growth, we have opportunities with our Waterloo-Wellington division for NEW and EXPERIENCED Social Workers.


  • Use of your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the HCCSS referral and other sources
  • Function as a member of a therapy team who are accountable for the therapy care provided
  • Develop appropriate time-specific, measurable goals and outcomes in collaboration with the client and / or caregiver(s), and in keeping with the HCCSS goal for treatment
  • Committed to quality and accuracy of care
  • Provide Professional Clinical therapy services (disc. specific) for clients in the community, fostering client-centered care and consistent with the Standards of respective Colleges.
  • Provide Assessment, Intervention and Discharge Planning, within the Scope of Practice, utilizing standard and non-standard assessments and interventions which are reflective of Best Practice.
  • Provide appropriate documentation according to Professional Standards and HCCSS contract guidelines.
  • Communicates with HCCSS, according to its standards
  • Collaborates with other health care team members and participates in multi-disciplinary team / family conferences, as required.
  • Participates in Quality Management and Risk Management programs / activities and in other projects / committees and student preceptorship, as needed.
  • Demonstrates a clear understanding of Community Resources, including funding.

  • Possess Master’s Degree (or equivalent) degree in Social Work from a recognized university
  • Current, unrestricted license to practice with Ontario College of Social Workers and Social Service Workers
  • Proven computer skills, including e-mail, word processor and work scheduling systems
  • Must be able to communicate effectively in both oral and written form and must be fluent in English
  • Experience in providing service in a community care setting is preferred.
  • Must have a reliable vehicle and driver’s license
