  • Serve as the primary point of contact for residents or resident families regarding resident care. This includes :
  • Timely follow up on resident and family concerns; communicates and advises RN / LPN and HSD of concerns by telephone if not in the community as necessary.
  • Proactively schedule and send resident care plan reminders to resident or resident families.
  • Effectively communicate with residents, their family members, and their significant others.
  • Effectively communicate with physicians, other health care professionals, and health careprofessionals, and other involved parties as necessary.
  • Access residents to ascertain if needs are addressed and report pertinent observations and related recommendations to the Director of Health Services or the Health Services Administrator. This includes :
  • Conduct service plan review meetings (after initial) and make changes accordingly.
  • Ensure care plan is completed according to recommended agenda
  • Ensure that necessary pre-work is to be completed to ensure that a comprehensive and successful care plan occurs.
  • Complete resident temporary care plans as necessary.
  • Complete state required resident forms as assigned
  • Document in EMAR as necessary
  • Effectively utilize and interact with home health, hospice, and other outside agencies to manage resident care.
  • Schedule, market and conduct all resident, resident family or outside participant support groups.
  • Assist nurse and HSD with coordinating and completing resident move-in’s
  • Partner with Staffing Coordinator to recruit and onboard new caregiver team members.
  • Manage, mentor and coordinate Medication Aides and Caregiver workforce; Assure that health services caregiver team members provide optimal resident service on day-to-day basis.
  • Create new resident chart (includes set up and that resident pictures area taken and uploaded).
  • Complete quality assurance audits as assigned by the Health Services Administrator and / or the Director of Health Services
  • Ensures resident chart is organized / purged according to company policy
  • 7. Ability to follow given budget to purchase supplies for various levels of care.

    9.Must be able to meet and exceed Touchmark values and the Touchmark Gold Standard.

  • Perform other duties as assigned by the Health Services Director / Administrator.
  • Serves as role model for all employees by displaying responsible, positive, cooperative attitude.
  • Promote resident advocacy among all employees.
  • Maintain current professional license.
  • Maintain current CPR certification.
  • Attend education and training classes, including as necessary to fulfill state regulations.
  • Maintain appropriate professional appearance.
  • Maintain ability to lift 60 pounds.
  • Must be able to work on site and provide care directly to residents through clinical outbreaks of flu, norovirus and other diseases including pandemicsituations and as determined.
  • Plan, direct and assist with resident care activities.
  • Transcribe and implement Physician’s orders
  • Administer medications as outlined in the Medication Administration Policy and Procedure.
  • Perform all required treatments.
  • Exercise judgment and accept responsibility for keeping attending Physician informed of pertinent changes in condition of the Resident.
  • Contribute to unit operation in respect to orderliness, cleanliness, supplies and equipment
  • Participate in encouraging and developing a professional attitude for self and co-workers.
  • Promote a positive image with internal public (Residents, staff, Physician, consultants, volunteers) and external public (family members, community groups and organizations, public at large).
  • Participate and lead Quality Improvement initiatives with the campus of care
  • Perform all other duties, as requested by supervisor(s), with a positive attitude.
  • EDUCATIONAL REQUIREMENTS :

    1.Bachelor’s degree in health administration, social work, psychology, gerontology orrelated field. Previous social services experience preferred.

    Current ALF / RCFAdministrators license preferred.

    2.CPR / AED certification preferred.

    3.Eligible for continuing education credits and / or Best Friends Certification

    WORKING CONDITIONS :

    You will perform your primary duties in a multi-level, Continuous Care Retirement Community. Offices and the community are well lighted and ventilated.

    Sitting, standing, lifting and walking intermittently is normal. You will be dealing with the public, staff, visitors, family members and residents under all conditions.

    Position is subject to frequent interruptions. Potential hazards may include possible injury or strain from manipulating or moving equipment, moving resident, dealing with a combative / abusive resident, or possible exposure to communicable disease orinfection.

    Must be able to walk the full extent of the campus. Full time onsite physical presence is required for this position.
