Keep reading, we have an opportunity for you to apply your passion and expertise to this exciting Leader opportunity working with Primary Care Network leaders, local operational leaders, Project Managers / Coordinators, physicians and Division of Family Practice partners, corporate and support departments and Community Partners.

You will provide oversight, guidance and recommendations related to the planning, implementation and operationalization of Urgent Primary Care Centres (UPCCs) / Primary Care Networks (PCNs) across Fraser Health to ensure consistent and timely implementation.

This is a critical role for the success of the UPCCs and PCNs as you will be accountable to provide strategic direction, project management, leadership and oversight in accordance with the Ministry of Health policy, timelines and accountabilities.

We are looking for someone who has a good sense of awareness and sensitivity to the unique requirements for the various partner interests of the organization including Community partners.

Build on your career experience as you :

  • Utilize input from clinical and operational partners, develops models of Primary Care delivery that meet the needs of Fraser Health populations, including vulnerable populations
  • Develop, implement and lead a robust project management framework with templates, samples and guidance including methodology mentorship in alignment with FH project management philosophies, to support the development of UPCCs / PCNs
  • Manage the operationalization of the portfolio through the recruitment, retention and leadership of staff including assessing performance, discipline and termination of employment if required
  • Implement and oversee coordinated processes and quality assurance observation with partners, clinical leads and the Primary Care leadership team.
  • Review indicators and take action to ensure programs and services operate efficiently and effectively in collaboration with the requirements of the Primary Care leadership team and partners.
  • Develop processes and accepted guidelines and manages outputs of the team to ensure consistency and alignment with Fraser Health strategic imperatives and the Primary Care portfolio objectives.
  • Professional / Technical Capabilities :

  • Good knowledge of project management principles and methodologies coupled with an in-depth understanding of the health care system at both the strategic and operational levels.
  • Demonstrated knowledge of primary health care and healthcare delivery systems.
  • Ability to operate a standard PC and effectively utilize a variety of software applications to support the initiative management activities.
  • Are you motivated to join our team? Here is what we would like you to have :

  • a Master’s Degree in Business Administration or Health Care discipline,
  • 7 years recent leadership / management experience, including a minimum of 5 years in a project or change management role;
  • An equivalent combination of education, training and experience.
  • Valid BC Driver's License and access to a personal vehicle for business related purposes.
  • We have 2 Regular Full Time opportunities located at Central City in Surrey, B.C . This role will require travel to various communities within Fraser Health.
