Build on your career experience as you :

Provide expertise and leadership to Fraser Health Specialized Community Services Programs. This will include coordinating the clinical requirements for Assisted Living services to ensure clinical and quality care standards are maintained.

You will collaborate with the Manager, Assisted Living Services, work with service providers to ensure the consistent delivery of safe and appropriate quality care to clients.

This leadership role will allow you to lead regional internal and external meetings to engage partners, identify and resolve clinical issues, and ensure consistent delivery of service.

Bring your leadership skills and expertise to :

  • Provide expert consultative advice to clinical leaders, health care teams, and other health care practitioners in regards to the provision of care within Assisted Living sites, within the parameters of service contracts.
  • Own the coordination of complex admissions;
  • Engage Home Health staff, contracted service providers, and hospital staff to develop plans while ensuring eligibility requirements and quality standards for Assisted Living are met and maintained.
  • Act as a key resource to clinical teams and relevant Assisted Living clinical operations across Fraser Health.
  • Ensure quality clinical standards are communicated and reviewed.
  • Identify risks and develop actions sought at helping teams meet regional Assisted Living service standards and quality indicators.
  • We will be looking for you to have the following :

  • A Bachelor's degree in a health related field;
  • 5 to 7 years recent, related clinical experience including 2 years in a leadership role, or an equivalent combination of education, training and experience.
  • This regular full-time position is based in New Westminster, B.C.
