The Pharmacist is involved in clinical activities as an integral member of the healthcare team. Patient-centered pharmaceutical care, education and evidence-
based research into patient care are all major foci. Activities include pharmaceutical assessment, management and evaluation.
The Pharmacist will participate in interdisciplinary patient care rounds, provide patient-centered medication therapy management and make appropriate evidence-
Participates in mentorship and professional development of Clinical staff including pharmacy residents and pharmacy students.
Precept residents and students in rotations of area of specialization. Engages in professional self-development; Collaborate with Critical care team to develop intervention strategies to maximize medication utilization, including but not limited to evidence based cost effective medication guidelines, clinical pathways, core measures, best practice guidelines, and policy development related to medication management in critical care.
Schedule Work Hours
Monday’s and Tuesdays 7.5 hour shifts in CCU and F3.

1. Licensed by the Ontario College of Pharmacists or eligible for license
2. Baccalaureate of Science Degree in Pharmacy
3. Minimum of 2 years experience in a hospital setting
4. Demonstrated computer skills including pharmacy systems, word processing, spreadsheet and email systems
5. Demonstrated ability to function as an independent practitioner
6. Demonstrated communication, teaching, teamwork and evaluation skills
7. Demonstrated commitment to ongoing learning
