DEPARTMENT : Complex Continuing Care- 2CRATE : $22.01 to $22.44 hourly JOB CODE : 5030.8 SHIFT : Shift Work DUTIES : 1. Report any changes in patient condition or occurrences / incidents immediately to the nurse.

4. Ensure dirty linen is placed in the dirty utility room. 5. Assist with patient admissions, transfers and discharges. 6.
Operate mechanical lifts and utilizes assistive devices as directed. 7. Assist with collection of basic patient data which includes : height, weight, and measurement of intake and output.
May take and record vital signs once education package has been done. 8. Collect urine, stool or sputum specimens as requested.
9. Assist with preparation of patients for transport. 10. Provide constant care or close observation to patients as directed.
11. Answer telephone and patient’s call lights and locate nursing staff as necessary. 12. Speak with patients and families and communicates pertinent information to appropriate nurse.

21. Assist with patient care duties on other units, as needed. 22. Other duties as assigned. QUALIFICATIONS : 1.Personal Support Worker certification from an approved college program.
KNOWLEDGE : 2.Knowledge of patient care requirements and equipment specific to the clinical area. 3.Computer literacy in a Microsoft environment.
ABILITY : 4.Ability to meet physical demands of the job, including extended standing, walking, lifting and climbing using proper technique.
5.Ability to work effectively and efficiently as a team member in a fast paced environment in stressful situations. 6.Ability to maintain successful working relationships with the interdisciplinary team to achieve positive patient outcomes and meet and interact with people in a pleasant, professional, responsible and reassuring manner.
