CoolAid Community Health Centre (CACHC) is a dynamic, community-based health care centreserving Greater Victoria. We offer a broad range of services, including primaryhealth care, dental care, harm reduction, acupuncture, dietician, health promotion,counselling, STI testing, Hep C treatment programs, and OAT / PPRM.

We arecommitted to serving people who face barriers to health care, and we strive toreduce these barriers. Cool Aid Society is a multi-site organization.

Reportingto the Director of Health and Support Services, the Social Worker willparticipate in community development work with clients, staff, and otherorganizations to develop or strengthen the appropriate resources and policiesfor this population.

The Social Worker works within an integrated,interdisciplinary primary health care team serving a population with high ratesof trauma, mental health and substance use challenges, and who may also behomeless or marginally housed.

The successful candidate will workcollaboratively with the Health Centre Team and the Outreach Health Team andwill participate in ride-alongs with the Mobile Clinic, splitting their timebetween the Health Centre and Health Outreach.

Specific Responsibilities :

  • Completessocial histories and assessments to identify needs; formulates and implementsconsultative social work services and supports.
  • Providesclient-centered case management and coordination of services to overcomebarriers to services and to promote equitable treatment for all people.

  • Developscomprehensive case planning and service co-ordination.
  • Consultswith other health care team regarding psychosocial aspects of the client’ssituation.
  • Worksclosely with the OAT / PPRM prescribers to provide immediate crisis supports andcase management to ensure appropriate navigation and access to services andresources.
  • Maintainstimely case records, using electronic medical records to assist otherpractitioners in continuing client care.
  • Maintainsongoing communication and case management with the Ministry of SocialDevelopment.
  • Liaiseswith social and health services in the community as the role of the CHC SocialWorker.
  • Facilitatesaccess to service and participates in individual advocacy for clients regardingissues such as housing, income support, food, clothing, health, employment, andlegal services.
  • Liaiseswith community agencies and institutions regarding issues relevant to clientneeds.
  • Participatesin the overall activities of the Health Centre, e.g., in-service training,committees, special events, shared staff responsibilities.
  • Providesconsultation to other CHC staff on the psychosocial aspects of clients’functioning.
  • Assistspatients in applying for and maintaining eligibility for benefits such asIncome Assistance, CPP, rental subsidies, and develops good workingrelationships with stakeholders who provide these benefits.
  • Assistingwith the completion of PWD (Persons with Disabilities) applications.
  • Participatesin continuous quality improvement by conducting audits, completing appropriatereports, monitoring clinical indicators and satisfaction surveys, providingfeedback, following up on risk factors, and making recommendations forimprovements.
  • Mayinclude some accompaniment with clients to appointments related to key systemsnavigation (i.e., court appearances, systems appointments, etc.)
  • Qualifications : Education


  • Masterof Social Work degree (MSW) from an accredited school of Social Work.
  • Registeredwith the BC College of Social Workers (BCCSW).
  • Experience

  • Aminimum of three years in the health / social services field. Particularly,working experience with individuals and groups who have concurrent mentalhealth and substance use issues, who have faced or are at risk of homelessness,have criminal justice involvement, etc.
  • preferably in a community healthsetting.

  • Aminimum of three years of case management / service navigation and crisisintervention experience, preferably in a community health setting.

  • Completethe Ministry of Public Safety and Solicitor General Criminal Record Check.

  • Hourly Rate $35.65to $44.56

  • Work Days andTimes Monday to Friday 8 : 30am to 4 : 30pm.
  • WeeklyHours 35 hours per week.
  • Union Exempt.
  • Benefits Eligiblefor 100% employer paid benefits including pension, health, and dental care.
  • Job Type Permanent Full-time.
  • As of November 1, 2021, Cool Aid requires that all its staff are fully vaccinated (7 days past their second dose of vaccine) against COVID-19.
