Position : CDE Pharmacist, FT Regional, 1 Year Contract, GTA+

LMC Pharmacy is a group of diabetes specialized pharmacies co-located in LMC Diabetes & Endocrinology Centres. Our main activities are consultations, including diabetes medication and device training, patient education, comprehensive medication assessments, care plans and medication therapy management.

Our collaborative care setting includes interactions with endocrinologists, diabetes educators, health coaches, chiropodists, optometrists and research teams.

In addition, we are a dispensing pharmacy.

Currently, our growing team is looking for a CDE Pharmacist to join us on a Full Time Regional, 1-Year Contract basis in the Greater Toronto and surrounding areas (Barrie, Oakville) .

Job Responsibilities :

  • Reporting to the Pharmacy Director, the Pharmacist will support the pharmacy's patients by providing diabetes counseling and dispensing services
  • Provision of medication and device training, patient education, comprehensive medication assessments, care plans and medication therapy management via the Medscheck program
  • Provision of focused care to a rapidly growing base of patients with diabetes and other endocrine conditions
  • Support all responsibilities related to pharmacy operations including medication and supplies dispensing and shipment, as well as administrative tasks and reports
  • Requirements :

  • Part A licensed pharmacist with the Ontario College of Pharmacists
  • Current and valid Certified Diabetes Educator (CDE) designation
  • 4-5 years of Ontario pharmacy practice and diabetes education experience preferred
  • Must have previous retail pharmacy experience
  • Must be proficient in Kroll
  • Excellent verbal and written communication skills
  • Excellent interpersonal, organizational and time management skills
  • Ability to work independently (alone) or in teams
  • Valid driver's license and vehicle, as this is position requires travel to various sites
  • Availability Monday to Friday, between 830am - 530pm, although the majority of our sites have 9-5 hours
  • Fluency in Gujarati, Cantonese, Mandarin or Italian considered an asset
  • In addition to your CDE, having therapeutic certifications in other pharmacy consultative services considered an asset
