We currently have an exciting opportunity for you to bring your excellent work ethic as a dedicated Case Manager, Community Mental Health & Mentally Disordered Offenders within our Mental Health and Substance Use Services located in Abbotsford, British Columbia.
As a Case Manager you will work closely with other members of the multi-disciplinary team to provide clinical services to clients in Adult Community Support Services (ACSS) including services to Mentally Disordered Offenders (MDO).
Build on your education and career experience as you :
Function as a primary therapist and case manager, providing diagnostic client assessment and short and long term individual, group, and family therapy, using a variety of interviewing, counselling, and assessment methods.
Manage cases and formulate treatment plans and goals in conjunction with assigned agency staff, and other services including Probation Services.
Facilitate clients' utilization of appropriate community services and rehabilitative programs by performing duties such as researching available services, liaising with these agencies, providing related advice and consultation to clients and providing referrals to appropriate community treatment alternatives and services.
Utilize available community resources to develop supportive and rehabilitative programs for adults and their families.
Make arrangements for clients to be admitted to hospital when required, including on an involuntary basis and make phone calls to appropriate parties and necessary transportation arrangements.
Liaise with Forensic Community Outpatient Clinic and collaborates with crown counsel or judiciary regarding special court processes as required.
Maintain client records and statistics, prepares reports, and completes forms as required, in accordance with Fraser Health policies and standards, including clinical record standards of Mental Health Services.
Obtains consent forms for release of client information.
Participate in clinical research and program evaluation activities relevant to the needs of clients, families, hospitals, and the community, by providing information as requested.
To join our team you will have :
Master's Degree in Social Work, Clinical Counselling or other relevant health related discipline.
One (1) year of relevant clinical experience in either a hospital or community setting, including experience working with mentally disordered offenders, forensics or corrections, or an equivalent combination of education, training and experience.
Valid B.C. driver's license and access to personal vehicle for business related purposes.
