The Community Services Coordination Network is currently recruiting two highly motivated professionals to assume specialized responsibilities within the Children’s Service Coordination Team.

1 position - Oxford / Perth) & (1 position - Middlesex / Elgin)

Through a community based process, the Ministry of Children and Youth Services has selected the Community Services Coordination Network to implement and deliver a new Coordinated Service Planning model in London-

Middlesex, Oxford, Elgin, Huron, and Perth Counties. The new model is intended to improve the service experience and outcomes for children and youth with multiple complex special needs and their families.

The Service Planning Coordinator will be responsible for connecting these young people and their families, in a timely fashion, to the multiple services and supports they require in order to address their identified needs.

This may include services delivered in other sectors, various community based services, and the services of multidiscipline professionals.

The Coordinator will be responsible for developing, implementing and monitoring a single Coordinated Service Plan with each young person and their family that is responsive to the individual’s goals, strengths and needs.

The young people identified to participate in this specialized process may experience challenges related to multiple areas of their development including their physical, communication, intellectual, emotional, social and / or behavioural development, that are likely to require services from multiple sectors and / or professionals.

The ideal candidate will possess the following qualifications :

  • University degree or advanced education and experience in social services
  • A minimum of 5 years’ experience working in the area of children’s mental health and / or developmental services
  • A strong understanding of, and commitment to the service coordination model, the principles of person directed planning and the WrapAround process
  • Demonstrated ability to work collaboratively with individuals, families, service agencies, professionals and multidisciplinary teams
  • Proven facilitation skills are essential to this work, including the ability to develop, implement, and monitor coordinated service plans
  • Excellent interpersonal and written communication skills
  • Ability to work in an environment of change
  • Knowledge of and ability to access cross-sectoral community resources
  • Proven ability to work both independently and collaboratively
  • A valid Ontario Driver’s License, access to a reliable vehicle, and the willingness to travel and work flexible hours as required
