Your Opportunity :
As a Registered Nurse (RN) - Transition Services Intensive Case Manager you will be part of the Transition Services Intensive Case Management Team.
The scope of this new team is to facilitate earlier discharge from the acute care Emergency Department, increase the number of clients assessed for placement at home, and identification of complex clients to facilitate the most appropriate care stream / specialty program referrals.
The RN - Transition Services Intensive Case Manager is responsible for collaborating and organizing comprehensive health care services and care planning for clients in a variety of settings.
The RN Case Manager advocates for and balances the needs of the individual / family / community with organizational, legislative, and Alberta Health Services financial accountabilities.
This role will require sound critical thinking skills, unique service delivery approaches, and balancing ethical decisions in the clinical / home, supportive living (SL), long term care (LTC) and hospital setting.
The RN Case Manager will be conducting regular visits to the client in acute care, home, SL and LTC settings; as well as work collaboratively with acute care, Transition Services and care providers from Home Care / SL and LTC to facilitate earlier and smoother discharges.
This role will case manage clients up to 30 days once transitioned out of acute care. You will be required to utilize the applicable electronic systems to maintain the client health record.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-
being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families.
You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.
You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Required Qualifications :
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA).
Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications :
A strong clinical background in Nursing. Physically able to perform all assigned tasks.
Preferred Qualifications :
Baccalaureate Degree.Two years of experience as a Transition Services or Home Care Coordinator (last five years). Previous community experience an asset.
Excellent organizational, oral and written communication skills; high level of independence and decision-making ability.Previous experience working with a multidisciplinary team.
Understands and is able to identify standards and philosophical principles for Case Management.Demonstrated willingness to become proficient in use of the EHR.
