Responsibilities

Reporting to the Manager of Patient Flow and Medical Social Work, the Medical Social Worker will provide professional social work services to patients and their families.

Services may also include the provision of support groups. With considerable latitude and encouragement to initiate and work independently, the incumbent will be an adaptable and flexible person with the ability to work collaboratively in a variety of work settings.

Social Workers are expected to exhibit sound professional judgement and initiative while working within the framework and objectives of the Nova Scotia Health Authority.

Some home visits and travel are required.

Qualifications

  • Master’s Degree in Social Work
  • Registered or eligible for registration with the Nova Scotia Association of Social Workers
  • Minimum of three (3) years clinical experience in an acute care medical / surgical setting.
  • Evidence of extensive knowledge of community resources.
  • Evidence of ability to work independently with a multidisciplinary team as well as with both hospital and community based services / agencies
  • Evidence of ability to coordinate / develop / document and implement care plan for hospital discharge and safe transition back to the community or institutional setting.
  • Demonstrated use of innovative problem solving and application of evidence based practice to patient services.
  • Knowledge of Joint Discharge Operations Committee and its purpose.
  • Valid driver’s license and access to reliable transportation
  • Physical ability to perform the duties of the position
  • Good attendance and work record
  • Competencies in other languages an asset, French preferred
  • Successful Applicants are required to provide a criminal record check (including Vulnerable Sector Search) before starting employment and assume any associated costs as a condition of employment
  • Hours of Work

    Temporary Part-Time 0.8 Position; 60 hours bi-weekly

    With an approximate start date of November 5, 2018 and an expected end date of July 6, 2019 (approx. 8 months)

    These are anticipated dates and may be subject to change

    Travel between sites may be required.
