In accordance with established standards, professional requirements and as a member of the multidisciplinary health care team the Discharge & Flow Specialist provides administrative and clinical leadership to all staff in a supportive, empowering manner while facilitating the effective and efficient movement of admitted patients through the hospital system.
Responsible for hour-by-hour planning, organizing, directing, coordinating, and evaluating activities related to bed utilization and patient discharge.
The primary responsibilities include : participating in implementing and coordinating a patient-centered care plan with patients, family members, significant others, and the multi-
reviewing all incoming applications, monitoring chronic care, nursing home application; liaising and establishing contact with members of the multi-
disciplinary team and appropriate community agencies to obtain, provide or exchange information relating to patients’ psycho-
ensuring appropriate patient assignment based on clinical need, working with other departments to assess the need for standards to optimize the utilization of beds (inclusive of pre-
operative days and direct admissions).
