We have the perfect opportunity as a Clinical Pharmacist Specialist .

Fraser Health has recently implemented an innovative new strategy to deal with the ongoing opioid crisis, focusing on the stewardship of opioids used for hospitalized patients.

You will work in collaboration with physician champions, and you will be provided with the opportunity to take on a unique clinical pharmacy practice and participate in the continued development of this ground-

breaking program. This is one of two Clinical Pharmacy Specialist positions in opioid stewardship, one based at Royal Columbian Hospital and one at Surrey Memorial Hospital.

As a Clinical Pharmacist Specialist you will work to provide the following services to our patients & colleagues :

  • Consult with internal and external health care professionals on the application of advanced level of theory to delivery of pharmaceutical care.
  • Provide clinical direction on complex cases using evidence based evaluations to evaluate drug therapies and patient medical histories.
  • Participate in patient education and discharge consultations and provide recommendations in the planning and treatment assessment policies and procedures
  • Assist in the development of drug therapy guidelines for individual disease conditions.
  • Identifies and initiates clinical drug research.
  • Prepare items such as case studies and articles for submission to professional journals
  • Conducts pharmacological education or in-service sessions
  • Represents the department on multidisciplinary care committees, and at external events.
  • Additionally, we would depend on your expertise to ensure that our pharmacists have the most current information as they do their important work of validating prescriptions and identifying safety concerns.

    To join us you will need :

  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Completion of an accredited Hospital Pharmacy Residency Program
  • Post-graduate Doctor of Pharmacy Degree
  • Two (2) year's hospital pharmacy experience including a minimum of one (1) year experience in opioid stewardship
  • We are looking for a motivated individual seeking a new challenge in patient care delivery. The position requires strong communications skills, up-

    to-date clinical expertise, as well as an interest in implementing and evaluating evidence-based practice change. This position will be linked to all aspects of opioid use from acute pain to addiction issues including transition to the community.

    This will be pioneering work, as few opioid stewardship programs currently exist. The strategies implemented could set the stage for expansion of the program to additional sites.

    This is a regular full-time position and you will be based at Surrey Memorial Hospital .
